First-beds list

Quick prescreen

We are preparing to open. Prescreen a client now and you're first in line when beds open. Non-clinical details only.

Goes straight to Randy Godwin, Clinical Director. Prefer email? randy@bhofm.com

e.g., weekday mornings

We'll call you back to talk about the client. Please don't share any client names or health details here.

This form is for preliminary referral inquiries only and should not be used for emergencies, crisis communication or transmission of confidential medical records.

Why it matters

Where you place someone reflects on you.

A bad placement comes back to your desk as a readmission, a call from family, or a client who walked. We built a small, 10-bed home with awake staff all night, clinical leadership that answers the phone, and a team that tells you honestly when someone isn't a fit, so the people you send here stay, stabilize and step down on plan.

Admission criteria

Who fits, and who doesn't

A good fit

  • No suicide attempt, active suicidal or homicidal plan, or active intent within the past 30 days. Suicidal thoughts without a current plan or intent may be reviewed.
  • Medically stable; not in need of acute medical withdrawal management (detox) or skilled nursing care.
  • Willing and able to take part in an unlocked, voluntary recovery home.
  • Documented risk of relapse, or functional impairment in self-care, housing stability or medication adherence, within the past 90 days.
  • Needs 24-hour structure and supervision because lower levels of care (routine outpatient) have not been enough.

Not a fit for this setting

  • Active acute psychosis or dementia.
  • Active suicidal or homicidal intent requiring acute inpatient hospitalization.
  • Severe medical fragility requiring hospital-level nursing care.
  • Active acute withdrawal requiring inpatient medical detox.
  • A stage 3 or 4 pressure sore.

What to send

Referral packet checklist

Once we are licensed, a complete packet includes:

  • Demographic / face sheet
  • Recent history & physical (H&P), plus psychiatric and substance-use evaluation
  • Current Medication Administration Record (MAR)
  • TB clearance / health clearance
  • Primary care provider and/or psychiatrist: name and contact information, so we can coordinate care

Clinical documents go only through the secure channel we give you on the call. Never send them through this website.

Funding

AHCCCS and other coverage

Insurance credentialing, AHCCCS Complete Care plan contracting and private-pay arrangements are being established ahead of our opening. We'll go over the client's coverage with you on the call back.

We coordinate authorization with your team so nothing stalls at discharge.

The process

Three steps

  1. 1

    Send a quick prescreen

Referrer details, client age, funding and target date. No names, no diagnoses, no records.

  2. 2

Our Clinical Director calls you back

A call within 2 business hours to talk through fit against our admission criteria.

  3. 3

    Coordinate next steps

Once licensed and when a bed is available, clinical documents are exchanged through a secure channel and next steps are coordinated together.

Clinical leadership

Who you'll be working with

Headshot of Randall Godwin, M.Ed., LIAC, CHW

Clinical Director

Randall Godwin, M.Ed., LIAC, CHW

A Licensed Independent Addiction Counselor with more than 25 years in substance use and co-occurring treatment, Randy has served as chief operating officer of an Arizona psychiatric hospital, overseeing a $10M+ budget, and as a clinical director. Since 2014 he has been a commissioner for a national accreditation body, completing 50+ site reviews of behavioral health programs, and his audits of Arizona providers carry a record of zero deficiencies. Trained in CBT, DBT and trauma-informed care, Randy reviews every referral personally.

Headshot of Amber Hayes, MSN, AGACNP-BC

Director of Nursing

Amber Hayes, MSN, AGACNP-BC

A board-certified acute care nurse practitioner with a decade of hospital leadership, Amber has served as the lead provider for a Level 1 trauma service in Phoenix and managed overnight inpatient care for complex surgical patients. She has written clinical protocols, trained nurses and new providers, and run a post-discharge follow-up clinic, and brings that same rigor here: medication safety, health screening, close coordination with each resident's doctors, and the judgment to spot early when someone needs a higher level of care.
