Admission criteria
Upon licensure, these criteria (aligned with AHCCCS AMPM Policy 320-V) will guide review of medical necessity for BHRF level of care.
Clinical admission requirements
- No suicide attempt, active suicidal or homicidal plan, or active intent within the past 30 days. Suicidal thoughts without a current plan or intent may be reviewed.
- Medically stable; not in need of acute medical withdrawal management (detox) or skilled nursing care.
- Willing and able to take part in an unlocked, voluntary recovery home.
- Documented risk of relapse, or functional impairment in self-care, housing stability or medication adherence, within the past 90 days.
- Needs 24-hour structure and supervision because lower levels of care (routine outpatient) have not been enough.
Mandatory exclusions
- Active acute psychosis or dementia.
- Active suicidal or homicidal intent requiring acute inpatient hospitalization.
- Severe medical fragility requiring hospital-level nursing care.
- Active acute withdrawal requiring inpatient medical detox.
- A stage 3 or 4 pressure sore.
Admission decisions will be based on an individualized clinical review of these criteria, safety, available staffing and the facility's approved scope of services.
Submitting a referral does not guarantee admission or placement.
